Step 1: Complete and submit the online registration form on the Maryland Medical Cannabis Commission (MMMC) website. (First Icon Below | Page 1)

Step 2: Obtain a signature from a physician registered with the MMMC attesting to the State that you will benefit from the use of medical cannabis. It is up to you to provide independent evidence that you indeed do have one of the following conditions: Admitted into hospice or receiving palliative care,chronic or debilitating disease or medical condition or the treatment of a chronic or debilitating disease or medical condition that produces: Seizures, Severe or Chronic Pain, Anorexia, Cachexia, Wasting Syndrome, Severe Nausea,  or Persistent Muscle Spasms (First Icon Below | Page 2)

Step 3: The State requires a $50.00 fee to obtain an optional ID Card.
